Agencies are trying to break down barriers for those in need of a variety of social services support, but as the saying goes, “we don’t know what we don’t know”. We are trying to find the best way to find and connect these resources across the City to link people to appropriate support no matter where they live in Hamilton.

Goal(s): A completed literature review that will inform future decision making for service providers to connect and coordinate, allowing seamless connection to existing services.
